Skilled Nursing Care

Patients come to San Tomas Convalescent Hospital when their physical condition is generally stable and the intense nursing care provided in the acute hospital is no longer needed. During this transitional time, further treatment and recovery time may be necessary before patients can be safely discharged to their homes, Assisted Living Facilities, other Skilled Nursing Facilities, etc. 


Our staff of nurses, therapists (Physical, Occupational, Speech), nursing assistants, dietician, and others will help to meet those needs. We function as a team and have weekly care conferences where we discuss our patients' progress towards discharge. Patients and family members are always welcome to attend and participate. We are here for you and want to work with you to meet your goals.

 

Services Provided

Below is an overview of staff who will assist with your needs.
 

  • The Nursing Staff will perform an admission assessment and will assess your recovery needs daily. They will administer medications, provide treatments, and encourage your independence.
  • Patients may receive physical, speech and/or occupational therapy depending on your doctor's orders for your care.
  • Therapists may work with you to develop a treatment plan to reach your movement and activity goals.
  • The Activity Team provides emotional support and opportunities for creative expression, relaxation, and socialization.
  • A Case Manager or Social Worker will visit you to help you plan for your discharge.
  • Dietitians plan meals for maximum nutrition and enjoyment—and to meet the needs of those on special diets.
  • Environmental Services will clean your room daily.
  • All team members will work with you and your family to provide optimal care so that you can have a timely and well organized discharge.
